How to become a HETRA Student:

1.  Click on the link below, download and fill out the paperwork. 

2.  Mail the paperwork to:

3.  Once your paperwork is received one of our therapists will contact you to schedule an evaluation.  At this evaluation the therapist will then suggest which service will best suit your child.
